It is a fillable, and savable, pdf form.
For activities lasting over 72 hours a medical evaluation by a licensed health-care provider is now mandatory. In the medical evaluation section (B) there is a height/weight chart that MUST be strictly followed if the event will take the unit to where emergency evacuation is more than 30 minutes away by ground transportation. This form is for both youth adult.
